LUX MED team

The controller of your personal data is LUX MED sp. z o.o. with its registered office in Warsaw at ul. Postpu 21c, which processes your data in order to contact you for the purpose of allowing remote verification of your identity to give you full access to the Patient's Portal. We process data including your name, surname, date of birth, email address and/or telephone number (depending on the communication channel you choose). Your consent is of a one-off nature. Your data will be deleted from our database as soon as the process to give you full access to the Patient Portal is completed. Remember that if you are our patient, your data will be processed on the basis of separate laws.
