Package Graphite

The LUX MED Group has prepared the Graphite medical programme - an innovative package of basic medical care. Under the Graphite package, we offer low fixed costs and a system of co-payments for actually executed doctor's appointments. The package provides unlimited access to medical specialists (6 specialisations) for just PLN 10, as well as basic laboratory and diagnostic tests.

Services not covered by the package (including consultations with other specialists) are offered at a 10% discount. Detailed information on the package (click here).

Terms and conditions of joining the individual Graphite programme:

• the programme is intended for persons aged below 65,
  
• the contract is signed for a minimum of one year and after that time it may be terminated with 30-day notice. If the contract is not terminated, it shall be extended indefinitely,
  
• payments are made up-front for the entire year or in half-yearly or quarterly instalments, via the Internet (by bank transfer or by credit card payment),
  
• services under the package are provided at the LUX MED, CM LIM, Medycyna Rodzinna and PROMEDIS facilities,
