The insurance of the costs of the treatment abroad and the consequences of accidents for Key Clients

Providing You the complex medical care, not only within our country, but also abroad, together with Towarzystwo Ubezpieczeń Allianz Polska S.A. (TU Allianz) we have developed exceptional insurance conditions under which You receive a product which is unique as regards the scope of insurance and the limit of liability.

In the offered insurance cover, the following should be stressed: the broad scope of insurance and high limits of liability, as compared to the scopes You had in the previous insurance cover, and the fact that the insurance also covers:
- the risk connected with doing, among others, the following sports: skiing, snowboarding, windsurfing and diving without scuba;
- the costs of the treatment connected with pregnancy and complications during pregnancy to the 32nd week of pregnancy;
- complication of all the chronic illnesses.
We wish to stress that there is no age limit of the insurance and its maximum period during one stay abroad amounts up to 180 days - in all the countries in the world excluding the territory of Poland and the country of residence.

THE PROCEDURE OF THE ENTRY INTO THE INSURANCE CONTRACT

A condition of the insurance coverage is having the right to enter into the insurance, resulting from the signed contract and the owned product, and informing Allianz about the willingness to enter into the insurance.

In order to apply for the insurance the Clients are asked to fill in and send the attached form (download DOC file) in one of the following ways:
- by e-mail to the address: zgloszenia1@allianz.pl,
- or in the case of inability to send the declaration by mail - in writing to the address: Allianz Polska Services Sp. z o. o., Centrum Obsługi Klienta Ul. Chocimska 17, 00-791 Warszawa; with a note "The activation of the insurance of the treatment costs during foreign travels."

People who send the application to the 24th day of every month shall be included in the insurance cover beginning on the first day of the following calendar month. In the case of applications sent after the 24th day of the calendar month the insurance cover begins on the first day of the second calendar month following the date of receiving the application. "

In the case of the lack of rights to being included in the cover under the above insurance, You will receive from us, by the end of the month preceding the beginning of the insurance cover, information about the lack of cover, including the reason.
